About Heather L Grantz
About the Provider
Heather Grantz, AGNP, is a certified adult gerontology nurse practitioner. She earned her bachelor's degree in nursing from Grand Valley State University in Allendale, Michigan, and her master's degree in adult-gerontology primary care nurse practitioner from University of Michigan in Ann Arbor. Heather is a member of the American Association of Nurse Practitioners, the Michigan Council of Nurse Practitioners and the Hospice and Palliative Nurses Association.
Heather will provide you with specialized care – from the time of diagnosis throughout the course of your treatment – optimizing your quality of life by anticipating, managing and preventing suffering. She works with our expanded interdisciplinary team of experts including physicians, nurse practitioners, registered nurses, social workers and chaplains who partner with you and your referring provider to address the physical, intellectual, emotional, social and spiritual needs you may have throughout your illness.
